Educational Assistance Agreement

Hi, I have been asked to create an agreement form for educational assistance that states the employee that received aid will work with us for X number of years and if the ee doesn't, then s/he is responsible for repaying a pro-rated amount of money. I emailed you the one we use. Our policy does not go back more than one year of tuition reimbursement.
