We know we will have at least a 10% increase next year. The underwriter made an error when quoting us, but the company agreed to honor the quote. We pay 70% of the health insurance. Doesn't matter if ee only or ee plus dependents.

The majority of our employees are governed by DOT. DOT requires we provided dates of employment, accidents and drug results. Of course, we do require a signed authorization and liability waiver from the employee.

We do the same. You usually have all the necessary information in the comp file. No need to duplicate efforts.
